Vendor Registration Information

The following documents are required to be on file for all vendors wishing to do business with the City of Warner Robins. These forms must be completed to ensure your company is operating in compliance with the Georgia Security and Immigration Compliance Act OCGA §13-10-91. A W-9 Request for Taxpayer Identification Number and Certification must also be completed and returned. In order for the City of Warner Robins to do business with your company, you must complete, have notarized, and return these forms to our office:

• W-9 Request for Taxpayer Identification Number and Certification

• Georgia Security and Immigration Compliance Act OCGA §13-10-91 E-Verify Affidavit

• Sub-Contractor Affidavit O.C.G.A. §13-10-91(b)(3)

  • Note: This affidavit is only needed if your company will be using a sub-contractor

• Sole Proprietor Law OCGA 13-10-91 (b)

• Sole Proprietor Affidavit
